HighRidge Kids New Family Form

This form is to be filled out by new families checking into HighRidge Kids for the first time.

Consent for our Media Team to take photographs and/or video of your child engaging in HRK services for the purposes of Social Media/Advertising for HighRidge Kids and HighRidge Church.

Consent for our HRK Dream Teamer to change your child's diaper and/or assist your child in the restroom if needed. (All of our HRK Dream Team is background checked. Only females over the age of 18 change diapers or assist children in the restroom)
